PROGRAMS

Community Wellness Nursing

AHNW provides community-based wellness nursing programs that bring accessible preventive healthcare, education, and support directly into the settings where people live and gather.

Our wellness nursing model is designed to promote healthy aging, support chronic disease management, improve healthcare navigation, and provide individuals with a trusted nursing resource within their community.

Council on Aging Wellness Nurse Programs

Through partnerships with Councils on Aging, AHNW provides regularly scheduled nursing services that support the health and independence of older adults.

Our nurses offer a welcoming environment where participants can receive basic health screenings, discuss health concerns, ask questions, and connect with appropriate healthcare and community resources. Our wellness nursing model is designed to promote healthy aging, support chronic disease management, improve healthcare navigation, and provide individuals with a trusted nursing resource within their community.
